Case Management

As Case Manager I create, manage and monitor my client’s treatment plan and facilitate effective team communication, all the while supporting and inspiring my client. In conjunction, I work with family members to create a healthy productive environment so that my client can individuate and move forward with confidence and ultimately more independence.

Create, manage, and monitor clients treatment plan.
Bring together an effective treatment team.
Facilitate effective team communication to best support client.
Provide support to client to comply and achieve treatment goals.
Help client make informed decisions by acting as their advocate.
Support and inspire client to achieve wellness and autonomy.
Work with family members to create a productive environment for client to individuate and move forward with confidence and ultimately more independence.
